UN envoy: Without deal in Syria, think Somalia not Yugoslavia

The United Nations' envoy to Syria?Lakhdar Brahimi argued against hopes that the country could find stability by devolving into a set of smaller states.?

After a week of attempting to craft a peace plan that both President Bashar al-Assad and the Syrian opposition would agree to, the United Nations' envoy to Syria said the situation will not stabilize on its own and that a political deal is no closer.

?People are talking about a divided Syria being split into a number of small states like Yugoslavia,? Lakhdar Brahimi said, according to The New York Times.??This is not what is going to happen. What will happen is Somalization ? warlords."?

?The situation is bad and it?s getting worse,? Brahimi also said, according to Bloomberg Businessweek. ?I can?t see anything other than these two paths: Either there will be a political solution that will meet the ambitions and legitimate rights of the Syrian people, or Syria will turn into hell.?

He warned that the violence could claim as many as 100,000 lives in 2013.

According to the New York Times, Mr. Assad did not respond to Mr. Brahimi's proposals and a Syrian opposition leader declined an invitation to Moscow to meet with Russian officials. Sergei Lavrov, Russia's foreign minister, said Assad could not be convinced to leave the country, which the opposition has insisted is a precondition for talks.?

Speaking about the yawning gap that has to be bridged for the two sides to sit down for talks, CNN reports that Brahimi said, "The Syrians disagree violently. On one side, the government says we are doing our duty to protect our people from ... terrorists. On the other side, they say the government is illegitimate," Brahimi said. "They are not talking about the same problem. They are talking about two different problems."

Brahimi's comments came the day after what CNN said?might be the bloodiest day in the uprising?? on Dec. 29, at least 399 people were killed.

According to Reuters, Mr. Lavrov pinned the blame for continuing violence on the opposition, even though the US, European countries, and most Arab states back the opposition's demand that Assad's removal from power come first.?

"When the opposition says only Assad's exit will allow it to begin a dialogue about the future of its own country, we think this is wrong, we think this is rather counterproductive," he said. "The costs of this precondition are more and more lives of Syrian citizens."

But the Syrian opposition's calculus has changed over the last couple months. A string of victories has made it optimistic abut winning the war in the end, and therefore less flexible in negotiations, according to Reuters.

Regime still has strength

But despite their recent success, "the government still has the bigger arsenal and a potent air force. It controls most of the densely populated southwest of Syria, the Mediterranean coast, most of the main north-south highway and military bases countrywide," Reuters notes.?

Russia appears to be making an effort to secure a meeting, agreeing to meet the opposition representative outside of Russia if he insists. Bloomberg reports that, according to RIA Novosti, the foreign ministry said talks could be held in Geneva or Cairo instead.?

Meanwhile, Brahimi is rapidly losing ground support in Syria, Reuters reports.?

The envoy's credibility with the rebels appears to have withered. In the rebel-held town of Kafranbel, demonstrators held up banners ridiculing Brahimi with English obscenities.?

"We do not agree at all with Brahimi's initiative. We do not agree with anything Brahimi says," the rebel chief in Aleppo province, Colonel Abdel-Jabbar Oqaidi, said on Friday.

US oil, gas exploration rigs drop this week by 11

Houston: Oilfield services company Baker Hughes Inc. says the number of rigs actively exploring for oil and natural gas in the US fell this week by 11, to 1,763.

Baker Hughes, based in Houston, said on Friday that 1,327 rigs were exploring for oil and 431 were searching for gas. Five were listed as miscellaneous. A year ago, Baker Hughes counted 2,007 rigs.

Of the major oil- and gas-producing states, Alaska and West Virginia gained one rig each.

Texas declined by eight rigs and California and Louisiana each dropped two.

Arkansas, Colorado, New Mexico, North Dakota, Oklahoma, Pennsylvania and Wyoming were unchanged.

The rig count peaked at 4,530 in 1981 and bottomed at 488 in 1999.

The fiscal cliff, Hawaii?s post-Inouye politics and 2013 predictions: Interview with Tiffany Au - (Making Waves: A Hawaii Perspective on Washington Politics)

HONOLULU, December 29, 2012 ? The whirlwind year that some prophesized to be the end of the world is now almost over and for better or worse, Planet Earth and all of us are still here. To give us a capstone perspective on the biggest issues and the year in review, I sought out former 2012 Republican House candidate and reigning Hawaii Narcissus Queen Tiffany Au.

Au, a second-generation American whose parents were Vietnamese boat refugees who fled to the United States, is perhaps the most unique personality I have interviewed this year and truly my column's featured "wavemaker" of the year.?

Danny de Gracia: So 2012 has been a long and very newsy year. President Obama got re-elected, the world didn?t end on December 21st, Hawaii has a completely new congressional delegation and Brian Schatz is now our new senator, filling the vacancy created by Dan Inouye's passing. What?s your thoughts on this year in review?

Tiffany Au: (Laughs) This was certainly a watershed year of key events in both local Hawaii and American history. We?re in an ongoing recession and the decisions that are being made in Washington ? I?m not sure if it?s necessarily the best course for the future and that the public is fully engaged or aware of what the consequences are going to be of this election.

I think the public sometimes confuses a good marketing strategy with having a competent leader. President Obama had a great campaign that was something to everyone but the implementation and continuation of his polices and their impact is going to affect the public in ways that will ultimately affect the taxpayer?s pocketbook and in ways people don?t realize.

Here in Hawaii, on Oahu the mayoral election brought out a very important issue to the front and that?s rail. I think its something that has the potential ? if not managed wisely ? can affect the economy. Hawaii relies on tourism and the military. If we have a rail that?s being built for 20 years, how will that impact our economy? Unfortunately the anti-rail candidate didn?t win, so now we?re going to see whether or not the construction of this project is going to bring about the great promises ? or not ? that the pro-rail candidates made.

Regarding the passing of Senator Inouye and Brian Schatz, I know many people in Hawaii didn?t take it well. A lot of people felt that Inouye?s wishes should have been honored because he?s been in office for over fifty years.

DDG: You?re referring to the memo that was delivered requesting that Rep. Colleen Hanabusa replace him.

Au: Right. As for how Senator Schatz will do in office, I?m not going to pass judgment on that, we?ll see how things go and how it turns out.

DDG: Well let?s look at this from a different perspective. When Governor Abercrombie appointed Schatz, the Hawaii Republican Party put out this press release saying ?

Au: Right, I saw that.

DDG: ? they were basically saying in effect that had Rep. Hanabusa been nominated they were ready to field a candidate for a special election and my first thought was why say that? Isn?t that like saying their electoral victory depends on the governor giving them a free throw? Isn?t that showing their hand a little too much?

Au: (Laughs) Oh. I saw that press release. My first thought was that its reactionary. It doesn?t promote action on behalf of the party.

DDG: That press release basically seemed ? I don?t know, it makes it seem like this is the end of the Hawaii Republican Party as a viable organization. That pretty much seems that you?re in a desperation mode. What do you think?

Au: I think that nothing really dies. Life is always in flux, so are organizations. I don?t think that the Republican Party can ever really die, and I also don?t think that the Democrats want the GOP to die. If that?s the case, there?s no way that they can blame anyone but themselves. They would be accountable for everything. It?s in the Democratic self interest to have a couple. Now the public is unaware?

DDG: Whoa. So you?re basically saying that (laughs) that it?s good to have controlled opposition?

Au: (Laughs) From a strategic standpoint the Democrats would always need to have a scapegoat.

DDG: You mentioned economics and the fallout of public policy. Let?s step into an alternate reality where you?d actually won your election to the House. What would you be doing right now policy-wise and what issues would you triage right now?

Au: Well the economy needs to be addressed. Pensions are pretty much not a guarantee anymore, the low interest rates are bad for people who want to save for retirement and combined with inflation, it?s a stealth tax on people?s future. The point that I?m getting at here is that you need a stable currency and you need some kind of predictability with public policy so that investment and wealth generation can actually occur. The American Dream is ultimately about property ownership and freedom to enjoy the fruits of one?s labor.

If I were in office the first thing that I?d be doing is going after the fiscal and economic policies that are causing the very distortions in the marketplace that are behind this slump we?re in. Right now people in elected office really don?t seem to get it when it comes to economic issues and that?s something that needs to be focused on. Hawaii is one of the worst places to do business. The cost of doing business here is so high. Gas prices are so high. There?s this inflationary cycle going on and people are going under and getting laid off. I?d work to not only fix that but the root issues behind that.

DDG: So what do you think is going to happen with the fiscal cliff?

Au: The fiscal cliff is (laughs) very scary. We?re probably going to see taxes go up. And with all that?s going on right now, it?s almost like the average American has no one speaking up for them and it?s every man for themselves. I think that a lot of people are concerned and confused in Washington about how to approach this. I think they?re putting politics before economic reality.

During the election here in Hawaii, I don?t know of that many people who even touched the fiscal cliff as an issue. We need to get our fiscal house in order. I ran as a Republican because I firmly believe that at the end of the day all these ideas that legislators come up with is impacting someone?s life and isn?t free.

The American people are hardworking, innovative and responsible. If you unleash them and allow them to get back to work, America will start working again too.

DDG: Right. Speaking of working, just recently across the Atlantic the party leader of UKIP and Europarl MEP Nigel Farage put out a message for the new year in which he said ?we?ll go on battling for no tax on the minimum wage to give the people in this country in low paid jobs a real incentive to get off benefits?. I heard that and I thought to myself, that?s a brilliant idea, how come we can?t do the same thing here? Why don?t we just repeal the taxes on the people making minimum wage here in the United States to encourage people to work and save. What do you think about that idea?

Au: People don?t grow unless they?re in a position where they need to work, so working is good. No income tax? That is a very interesting idea, because we need taxes for the roads, for public safety and basic functions of government and I?m not sure if some people who do work who make a little bit more that minimum wage would like that idea. There have been proposals over the years to just have a flat tax and we all pay the same taxes instead of having tax brackets, but we?re in an era where people know they have to work. There?s no getting around it.

DDG: Well, you know but then it becomes a question of at what point does working stop being about working to live and working to be in compliance to government and to pay for the government?s growing mission creep. I understand some level of taxation is going to be inevitable. But aren?t we getting to the point where we?re basically Duracell batteries now and the only purpose of our existence is to provide ongoing revenue streams to a government machine?

The point of working should be to live, to support your family, to enjoy life and the fruits of your labor. Not to pay for legislators who go on vacation after getting elected, not to pay for every idea that floats through a legislator?s mind.

Au: I?ve mentally prepared myself for a long haul (laughs) to do what I need to do to survive in this world. And so I know that life is hard and I know a lot of my classmates have moved out of the country to find work in places like China which has a better economy or are harnessing their skills in creative ways off-shore to launch themselves. With all these economic policies that penalize you ? increases in taxes, the individual healthcare mandate ? it?s going to get that much harder and people are going to have a different viewpoint of the government.

DDG: Well, the very reason that you?re here in Hawaii is because your parents fled from Vietnam because they didn?t want to get stuck in a country that had undergone a communist takeover. But on the flipside of that, I?m beginning to wonder if your parents jumped out of the frying pan and into the fire. Ronald Reagan famously said that people came to America for freedom but if freedom falls in America there?s no place you and I can go to.

Again, that?s why I?m asking, have we really reached the point where we?re just a revenue stream for the government and that?s our only reason for existence? Is this the Matrix where we live to power a government machine and pay for Congress? bills?

Au: We are turning into that Duracell battery. And you know in part it?s a function of the loss of understanding of the dangers of government growth. When I was growing up I read the dystopian books and essays that warn of the dangers of government like Animal Farm, Fahrenheit 451, A Brave New World, Nineteen Eighty-Four, we?

DDG: Did you, uh, read Obama?s book?

Au: Obama?s book?

DDG: The Audacity of Hope.

Au: Oh no, no, no, (laughs) we didn?t have that literature yet. But I had fabulous literature to draw upon. Ayn Rand?s Atlas Shrugged and the theme of these books is that if you?re being controlled by the government then the result is that you are just a pawn and not in control of your destiny. You are just this Duracell battery that you referred to.

And yes, we?ve entered into this era where everything is breaking down and the school system is not addressing the dangers we face today. People are not taught to be free and to think like free people, with respect to the right and proper role of government. People think money just comes out of plain air. The whole culture doesn?t have an understanding of how things should work. Conservatism is starting to be viewed as backwardness.

Look at pop culture. You don?t hear rappers singing, ?Oh gosh, I gotta pay my bills.? (Laughs) ?I need work hard, I have to make something.? Oh no, it?s just the end result. ?Let?s just have a good time? is the message.

DDG: ?Man I feel like money is attracted to me!?

Au: Right! But the key thing here is people don?t question motives anymore of government. They just want to feel good but in the end they?re no longer in control of themselves. People aren?t learning what freedom is anymore and knowing that there are consequences.

DDG: Yeah. Getting back to Hawaii, one of the things that people are upset about is the Public Land Development Corporation. Do you think in 2013 the PLDC will be repealed?

Au: Yeah, that is a big issue and a lot of talk for it to be repealed. That was even on the candidate questionnaires we did. The whole process and how it was done wasn?t right and a lot of people were upset. So hopefully it will be repealed. As I said there? a lot of talk but we need people to pay attention in the first place and be aware of what?s going on.

DDG: Wow. Well the City and County of Honolulu in February 2013 is going to be taking applications for the Neighborhood Board election. Are you going to file as a candidate? I think you?d be a shoo-in for that and you could continue to champion the issues.

Au: Honolulu Neighborhood Board? Well, I?d have to look into that. (Laughs) I did get that e-mail. I am in a transitional phase and there?s only so many things I can handle as one person. Right now what I?d really like to do is be able to take care of my family, my friends and myself and then I can inspire others. I think with Neighborhood Board that would be a lovely idea but I just have to see how things go in the coming days.

DDG: So tell our readers what your predictions are for 2013.

Au: I think as a positive the era of entrepreneurship is here because there?s a heightened level of risk that puts people into difficult situations but they?re also going to find innovative solutions to them. People are going to reinvent themselves and their circumstances.

On a negative, we also do have a situation now where there?s so much information but where is the direction? Wisdom is the proper use of knowledge. There needs to be an effort to leverage wisdom to put an end to this chaos. We need real leaders and real direction. I think in 2013 there are going to also be a lot of people who won?t know what hit them.

DDG: So. In review. Let?s talk the tops and bests of 2012. What was the best movie of the year?

Au: (Laughs) With the campaign I didn?t get to watch a lot of movies, but I liked 2016: Obama?s America.

DDG: So why didn?t the world end on December 21st?

Au: I was not afraid at all. Actually in the Mayan culture the end of the calendar doesn?t really mean the end of the world, it just means the end of the cycle.

DDG: So it?s like a pull and tear wall calendar, they forgot to load the next calendar stone or something.

Au: (Laughs) Basically. I wasn?t afraid. People used it as an excuse to have block parties.

DDG: So what was the top news story of 2012?

Au: Well, in Hawaii, it was the rail project.

DDG: Okay. So did you get what you wanted for Christmas? Aside from being elected to office. (Laughs)

Au: Thanks for rubbing it in! What I really wanted for Christmas ? oh gosh, now this is going to sound like a beauty pageant ? but what I really wanted for Christmas was world peace. (Laughs)

DDG: Have you seen Miss Congeniality? Maybe you should have asked for tougher sentences for parole violators. Now that?s true Republican style. (Laughs)

Au: (Laughs) Well I did at one point win Miss Congeniality.

DDG: I?m going to give you the last word of 2012 with my column, is there anything you?d like to say to the nation and to Hawaii?

Au: It?s important to remember above all else to keep on living and to keep on pressing forward. A lot of people aren?t sure what?s going to happen with the fiscal cliff and they?re afraid. But know that you?re not alone. All of us are in this together. One of the things that I?ve learned from Chinese culture is that patience is important in achieving your goals, so no matter what happens, no matter what you see, hang in there. Our attitude may be all that we have left.

Winston Churchill once said that history is written by the victors. Today perhaps our message is that history shall be written by the survivors. I want everyone reading this to know that we have to stay positive and to keep seeking the truth and to keep moving for a better tomorrow.?

ITC Judge recommends Samsung post 88 percent value bond, import bans in Apple patent case

If you're keeping track of the multiple, and let's face it, tiresome Samsung / Apple patent debacle, a document that just turned up at the ITC might spell more trouble for the Korean manufacturer. It's a publicly redacted version of Judge Pender's recommendations, and pertains to the October ruling that deemed Samsung borrowed four of Cupertino's designs. The most iconic being design patent D618,678 (that which you see above). The others include multi-touch patent 7,479,949 (which was tentatively invalidated) along with two other patents (RE41,922 and 7,912,501) relating to graphic display elements and audio hardware detection. If the recommendations are adopted -- and FOSS Patents suggests this is entirely possible -- Samsung could face a US import ban after a 60 day presidential review, an order prohibiting "significant" sales of infringing products in America along with a posting a bond for 88 percent of the entered value of mobile phones (plus 32.5 percent for media players and 37.6 percent for tablets) that include the breaching design features. Pender has, however, reportedly cleared several Samsung "designarounds" which, if implemented to satisfaction, would mean the tech giant could continue trading. For now though, the recommendations are awaiting the Commission's review.

No ethics breaches found in Countrywide VIP loans

WASHINGTON (AP) ? The House Ethics Committee said Thursday it found no violations among House members whose mortgage loans went through the VIP section of the former Countrywide Financial Corp., the company whose subprime loans helped cause the foreclosure crisis.

The committee said nearly all the allegations of favored treatment involved loans that were granted so long ago that they fell outside the panel's jurisdiction. The committee added, however, that participation in the VIP program did not necessarily mean borrowers received the best loan deal available ? and most lawmakers were not even aware they were placed in a VIP unit.

The actions of unnamed House staff members were harshly criticized. Emails indicated they reached out to Countrywide lobbyists for assistance with their personal loans, but those actions also were too old to remain in the committee's jurisdiction. The panel said that if the incidents had been more recent, the staff members could have faced discipline.

Countrywide was taken over by Bank of America in 2008. The House Oversight and Government Reform Committee reported in July that Countrywide made hundreds of discount loans to buy influence with members of Congress, congressional staff, top government officials and executives of troubled mortgage giant Fannie Mae.

The Oversight report named six current and former members of Congress who received what Countrywide referred to as discounts. All of their names had surfaced previously.

The committee has no jurisdiction over actions that occurred more than six years prior to the current Congress ? which began in January 2011.

But even if the statute of limitations had not run out, the committee said, inclusion in the VIP program was not by itself a violation of House rules or laws. The panel said it found that Countrywide's "discounts" applied to standard loan rates that were commercially available elsewhere.

"They are not the kind of 'gift' which would be, in and of itself, outside the realm of reasonable market rates for commercially available loans," said the statement issued by Ethics Committee Chairman Jo Bonner, R-Ala., and ranking Democrat Linda Sanchez of California.

Even so, the committee said, members must take steps to ensure they are being treated no differently than a member of the public who is similarly situated.

The committee statement added, "Of greatest concern...was email evidence regarding the specific conduct of some employees...who may have reached out to lobbyists or other government affairs officials at Countrywide for assistance with their personal loans."

Poor reading skills tied to risk of teen pregnancy

NEW YORK (Reuters Health) - Seventh grade girls who have trouble reading are more likely to get pregnant in high school than average or above-average readers, according to a new study from Philadelphia.

Researchers found that pattern stuck even after they took into account the girls' race and poverty in their neighborhoods - both of which are tied to teen pregnancy rates.

"We certainly know that social disadvantages definitely play a part in teen pregnancy risk, and certainly poor educational achievement is one of those factors," said Dr. Krishna Upadhya, a reproductive health and teen pregnancy researcher from Johns Hopkins Children's Center in Baltimore.

Poor academic skills may play into how teens see their future economic opportunities and influence the risks they take - even if those aren't conscious decisions, explained Upadhya, who wasn't involved in the new research.

Dr. Ian Bennett from the University of Pennsylvania and his colleagues looked up standardized test reading scores for 12,339 seventh grade girls from 92 different Philadelphia public schools and tracked them over the next six years.

During that period, 1,616 of the teenagers had a baby, including 201 that gave birth two or three times.

Hispanic and African American girls were more likely than white girls to get pregnant. But education appeared to play a role, as well.

Among girls who scored below average on their reading tests, 21 percent went on to have a baby as a teenager. That compared to 12 percent who had average scores and five percent of girls who scored above average on the standardized tests.

Once race and poverty were taken into consideration, girls with below-average reading skills were two and a half times more likely to have a baby than average-scoring girls, according to findings published in the journal Contraception.

Birth rates among girls ages 15 through 19 were at a record low in the U.S. in 2011 at 31 births for every 1,000 girls, according to the Centers for Disease Control and Prevention. But that rate is still much higher in minority and poorer girls than in white, well-off ones, researchers noted.

And in general, it's significantly higher than teen birth rates in other wealthy nations.

Teen pregnancies are a concern because young moms and their babies have more health problems and pregnancy-related complications, and girls who get pregnant are at higher risk of dropping out of school.

Upadhya said the answer to preventing teen pregnancy in less-educated girls isn't simply to add more sex ed to the curriculum.

"This is really about adolescent health and development more broadly, so it's really important for us to make sure that kids are in schools and in quality educational programs and that they have opportunities to grow and develop academically and vocationally," she told Reuters Health.

"That is just as important in preventing teen pregnancy as making sure they know where to get condoms."

Obama, Congress set for one last effort on "fiscal cliff"

WASHINGTON/HONOLULU (Reuters) - President Barack Obama is due back in Washington early Thursday for a final effort to negotiate a deal with Congress to avert or at least postpone the "fiscal cliff" of tax increases and government spending cuts set to begin next week.

No specific bill dealing with the cliff was on the schedule of either the U.S. Senate or House of Representatives, which are expected to return on Thursday after the holiday break. Investors are closely watching the talks, concerned that going over the cliff could throw the economy into recession.

Aides and members of Congress have said that a modest, last-minute measure to avoid the spending cuts and most of the tax hikes could pass the Democratic-controlled Senate if Republicans agree not use a procedural roadblock known as a filibuster, a commitment that Senate Republican leader Mitch McConnell has so far not made.

But to win approval in the Republican-controlled House of any bill that raises taxes on anyone, a rare bipartisan vote would be required. All 191 Democrats would have to team with up with at least 26 Republicans to get a majority if the bill included tax hikes on the wealthiest Americans, as Obama is demanding.

Some of those votes could conceivably come from among the 34 Republican members who are either retiring or were defeated in the November elections and no longer have to worry about the political fallout.

In the alternative, Congress could let income taxes go up on everyone as now scheduled and then during the first week of January, scramble and get a quick deal to cut them back except for the highest brackets, along with a measure putting off the $109 billion in automatic spending cuts that most lawmakers want to avoid.

Once the clock ticks past midnight on December 31, no member of Congress would have to vote for a tax increase on anyone - taxes would have risen automatically - and the only votes would be to decrease tax rates for most Americans back to their 2012 levels.

Obama and congressional lawmakers left Washington on Friday for the Christmas holiday with talks to avert the fiscal cliff in limbo.

The president will cut short his vacation in Hawaii and leave for Washington later on Wednesday, arriving in the capital on early Thursday.

Transfusions add risk in some heart attacks, finds study of patients with anemia

Dec. 24, 2012 ? When heart attack patients present in the emergency department with some degree of anemia, or anemic patients have a heart attack, physicians have a tendency, but not much guidance, about whether to provide a blood transfusion. The idea is that a transfusion could help more oxygen get to the heart. Recent national guidelines suggested that there simply isn't good evidence to encourage or discourage the common practice, but a new meta-analysis of 10 studies involving more than 203,000 such patients comes down on the side of it increasing the risk of death.

The next step for determining when the practice could be appropriate needs rigorous randomized trials that will generate more decisive, high-quality data, said lead author Dr. Saurav Chatterjee, a cardiology fellow at the Alpert Medical School of Brown University and the Providence VA Medical Center.

For the analysis published Dec. 24 in the Archives of Internal Medicine, Chatterjee and his co-authors combined and analyzed data from studies in which anemia patients with heart attacks either received "liberal" transfusions or received more restricted versions of the treatment or no transfusions at all. Liberal transfusions were defined as cases in which patients either received two units of blood or more or had a transfusion even with a hematocrit reading (a measure of red blood cell concentration) higher than 30 percent (normal is in the low 40s).

What the researchers found, after statistical adjustments to control for important medical factors, was that the risk of death was 12 percent higher for people who received the liberal transfusions than those who did not. Moreover, the group that received liberal transfusions had twice the odds of having another heart attack.

"What we found is that the possibility of real harm exists with transfusion," Chatterjee said. "It is practiced in emergency departments all across the United States. I think it is high time that we need to answer the question definitively with a randomized trial."

Of the 10 papers that Chatterjee and his co-authors reviewed, all but one were observational studies. The only randomized trial was a small pilot experiment.

Searching for an answer

Chatterjee began the study when he was a resident at Maimonides Medical Center in New York. He noticed a paper by the AABB (formerly the American Association of Blood Banks) in which the association said there was not enough clinical evidence for or against transfusions in heart attack patients.

For clinicians, the practice has always been a tough judgment call. Some transfusions are clearly necessary, for example when a patient's troubles include not just a heart attack but also severe ongoing bleeding, Chatterjee said. But transfusions also create health risks, such as an increase in potential clotting because platelets may clump together more, or from an inflammatory immune response to the introduction of blood of a "foreign" source into the body.

Chatterjee and his co-authors decided to comb the literature to determine whether, if properly combined and analyzed, existing data could provide some insight. They found 729 potentially relevant studies, but only 10 that had the right data to help answer the question.

Few as they were, Chatterjee said, the studies all told much the same story.

"One of the things that struck us is that there were very few studies in evidence of transfusion at all," Chatterjee said. "In our case, though, we found that the effect was pretty consistently harmful across the spectrum of studies, spectrum of time, and spectrum of patients that were enrolled in the individual studies."

Chatterjee said the study should not be taken to mean that transfusions should be stopped altogether for anemic heart attack patients. Instead, he said, doctors must continue exercising their clinical judgment, at least until results from a large, well-designed randomized trial can be produced. Mindful of the risk his study found, however, they might just want to shift their thinking about where the border is among borderline cases.

"Before a definitive trial is out there, we should be conservative, especially considering the high risk of harm," he said.

In addition to Chatterjee, the paper's other authors are Jorn Wetterslev of the Centre for Clinical Intervention Research in Copenhagen, Denmark; Abhishek Sharma and Edgar Lichstein of Maimonedes Medical Center; and Debabrata Mukherjee of Texas Tech University.
